Use the email address you registered with. Your password was sent in the welcome email on the first day of your cohort.

For the site owner. Authentication needs a server. Set data-endpoint on this form to your LMS login route, or replace the whole block with the sign-in shortcode from Tutor LMS or LearnDash. Do not store passwords in the front end.
